Programa de Transición a la Adultez
La transición de cualquier adolescente a la edad adulta tiene sus retos.
Descubrir quiénes somos y como participar en nuestras comunidades de una manera significativa son solo algunos de los caminos que podemos tomar en este período de transición. Trillium se ha asociado con organizaciones para crear programas adaptados a los jóvenes en el camino a la adultez, para ayudar a que estos caminos sean un poco más fáciles.
Los programas de transición que se ofrecen dentro de la red de proveedores de Trillium brindan una base sólida para fomentar la independencia, aprender habilidades esenciales para la vida y establecer conexiones sociales que son cruciales para el éxito en la vida. Afiliados entre las edades de 16 a 26 años trabajan con el personal para desarrollar habilidades de comunicación, aprender a manejar su tiempo y dinero, y comenzar a explorar carrera y escuela vocacional para después de la escuela. Esto puede incluir experiencias de aprendizaje basadas en el trabajo en entornos laborales en vida real para prepararse para futuras oportunidades en empleos competitivos.
Easterseals UCP
For adults with Intellectual and Developmental Disabilities (I/DD) of various ages. Participants focus on wellness, exercise, recreation, budget development, increasing social and independent living skills, volunteerism, fundraising, and public speaking. The Transition to Employment program prepares participants ages 16 to 26 with I/DD for employment. Participants focus on areas such as instruction and self-advocacy, job exploration, workplace readiness training, internship, and job placement opportunities. Work-experience activities take place both at the EMPOWER site, in the local community, and through self-employment activities.

Transition to Independent Living provides independent living support through coaching and monitoring technology to participants in the Transition to Employment program. The SensABLE Snacks program provides employment and volunteer opportunities to individuals with I/DD ages 16 to 26. Participants distribute food from local food banks in food desert areas in their surrounding communities. The trucks focus on three core values, employment, food access, and community. SensABLE Snacks was created in partnership between Easterseals UCP and Trillium Health Resources.
