Adult Transition Programs
The transition for any teenager into adulthood has its challenges.
Discovering who we are and how to engage with our communities in meaningful ways are just some of the paths we take during this transition period. Trillium has partnered to create programs tailored to young adults to help make those pathways a little easier.
Transition programs offered within Trillium’s provider network provide a solid foundation to foster independence, learn essential life skills, and make social connections that are crucial for success in life. Members ages 16 to 26 work with staff to develop communication skills, learn to manage their time and money, and begin career and post-high school vocational exploration. This can even include work-based learning experiences in real-world job settings to prepare for future competitive employment opportunities.
Programs are being delivered virtually or in-person for members with limited capacity due to COVID-19. Please contact the site directors below. Currently, all programs are enrolling new members except the Transition to Independent program in Ahoskie.
Easterseals UCP
For adults with Intellectual and Developmental Disabilities (/DD) of various ages. Participants focus on wellness, exercise, recreation, budget development, increasing social and independent living skills, volunteerism, fundraising, and public speaking. The Transition to Employment program prepares participants ages 16 to 26 with I/DD for employment. Participants focus on areas such as instruction and self-advocacy, job exploration, workplace readiness training, internship, and job placement opportunities. Work-experience activities take place both at the EMPOWER site, in the local community, and through self-employment activities.

Transition to Independent Living provides independent living support through coaching and monitoring technology to participants in the Transition to Employment program. The SensABLE Snacks program provides employment and volunteer opportunities to individuals with I/DD ages 16 to 26. Participants distribute food from local food banks in food desert areas in their surrounding communities. The trucks focus on three core values, employment, food access, and community. SensABLE Snacks was created in partnership between Easterseals UCP and Trillium Health Resources.

Autism Society of North Carolina
Transition to Employment (Greenville)
Trillium partnered with the Autism Society of North Carolina to offer a transition to employment program. The program is for people with autism between the ages of 16 to 28 years of age. This program is open to people of any ability. Participants cover independent living skills, social behavior, recreation, goal-focused environment, coping and self-regulation, transportation, and community integration skills, financial literature, job readiness, and internships for opportunities to job placement. Work experience activities are carried out both at the Transition to Employment site, in the local community and through self-employment activities.
