Choose Independence
Home is a place to make fond memories and surround ourselves with the things and people who bring us joy. We may live there alone or with others. Home can be in a house, an apartment, an assisted living center or other supervised setting. Most importantly, home is where we feel safe and most comfortable.
Grants for Independence
Trillium believes everyone has the right to choose where they want to live, whether they are living with a behavioral health or intellectual/developmental disability. Trillium Choose Independence program offers eligible individuals and families the chance to:
- buy equipment, supplies, and services that support living at home
- decrease the need for constant supervision,
- increase long-term success in living as independently as possible
Examples of items that may be available are:
- Durable Medical Equipment and supplies
- Assistive Technology Devices and services
- Home Modifications (i.e., fences, ramps)
- Medication Administration devices
- Tablets and appropriate apps when used as communication devices
- Diapers, Pull-Ups, and/or Incontinence supplies for people ages 3 and older
- Nutritional Supplements
- Memberships to community inclusion activities
- Smart Home Technology
Application Process
Trillium members over the age of three are eligible to apply for this grant. Please use the application form linked below. The application provides more details on all requirements. Applications are reviewed quarterly.
